  • We complete the VAC Mental Health / Psychiatric Questionnaire (Form 4016) and any related supporting documentation required by Veterans Affairs.

  • There is no cost to Veterans. All claim-related assessments are billed directly to VAC.

  • The length of the assessment can vary. It’s a multi-step process that includes completing intake and questionnaire forms, as well as meeting with several members of our clinical team.

    The total time depends on the complexity of your case and the requirements from Veterans Affairs Canada (VAC). We do our best to keep things timely and efficient, so you can focus on your recovery and next steps.

  • Yes — while in-person assessments are preferred whenever possible, we understand that some Veterans live far from major centres or face mobility or logistical barriers.
    In those cases, our team offers secure telehealth assessments using encrypted video conferencing to ensure privacy and accessibility.

    Virtual options are approved on a case-by-case basis and conducted with the same thoroughness, professionalism, and documentation standards as our in-person assessments.

  • Our team assesses a range of mental health concerns commonly related to service, including Post-Traumatic Stress Disorder (PTSD), depression, anxiety, operational stress injury, adjustment disorders, and moral injury.
    We also consider the impact of cumulative trauma, service-related stress, and transition to civilian life.

  • Once your assessment is complete, our Nurse Practitioner finalizes the report and ensures it is submitted securely to VAC or your Service Officer.
    You’ll also receive the opportunity to discuss next steps in care, including therapy, medical follow-up, or community support resources.
